Admissions Paperwork

Prior to admission, it is essential that all paperwork is completed and submitted to Grove School staff. This information is necessary to best serve your child.

The following information, included in your admissions packet, is required before or upon entry into Grove School:

    • Grove School Application for Admission

    • Letter from Current Therapist and Psychiatrist

    • Current photo of the child

    • Most recent psychological and educational testing

    • Grove School Admission Medical History

    • CT State Health Assessment Record (physical – within last 9 months)

    • Documentation of all immunizations

    • Proof of negative PPD test within the last 6 months

    • Grove School Admission Medication Orders

    • Emergency Information

    • The Grove School Admissions Permission Form

    • Credit Card Information

    • Copy of Insurance cards (front and back)

    • Prescriptions for ALL Medications signed by the current physician.

    • Most Current IEP from your school system

    • Current Transcript showing grades and earned credits

    • Health Insurance Portability and Accountability Act of 1996 (HIPAA)

    • Most Current IEP from your school systemRequest for Release of Information

    • General release form for medical, psychiatric, academic records, immunizations, psychological evaluation,
      educational evaluation, other

    • Transport release

    • Activity consent and release

    • Educational consultant release

    • Permission to release the CSP

    • Permission for therapist to discuss goals and objectives in LEA conferences

    • Sports and Activity waiver

    • Agreement to Assume Responsibility of Payment

    • Parent Directory Request

    • Photo release form

    • Permission to ski

    • Permission for music lessons

    • Permission to use the YMCA

    • Student Information Form

    • Includes request for release of school info


    • Health Insurance Claim Form (if submitting clinical costs to the insurance carrier)
